Does my daughter/son need a US Lacrosse Membership Number/USA Field Hockey Membership Number?

Yes, all players participating in any clinic, camp, or travel team must have a US lacrosse membership number (for all lacrosse related events) or a USA field hockey membership number (for all field hockey related events).  This is a yearly membership and is for insurance purposes only.  Please note, Medusa Lacrosse & Field Hockey LLC does not financially benefit from this $30 yearly membership.  Please use the links below to register/renew your membership.

What equipment does my child need for the Little Laxers program?

All girls (regardless of age) must have a women's lacrosse stick, goggles/eye mask, and a mouth guard. 

Boys ages 3 and 4 must have a men's lacrosse stick and gloves.

Boys ages 5 and up must have a men's lacrosse stick, gloves, helmet, and full pads.

All players should wear sneakers/cleats to each class.

We recommend asking for a "Beginners Stick" when purchasing equipment and a flexible mouth guard.

What equipment does my child need for the Field Hockey program?

All girls (regardless of age) must have a field hockey stick, shinguards, goggles/eye mask, and a mouth guard. 

All players should wear sneakers/cleats to each class.

All field hockey sticks are designed for the right side of the body.  There are no "lefty" sticks.

How Do I Pay?

Payments can be made by check or cash.  All checks payable to Medusa Lacrosse LLC.  All payments made in cash will receive a $10 discount.  Send in all payments to 82 Wyatt Road, Garden City, NY 11530 after you have submitted your child's form through our website. Please have your child's name on the envelope and/or check.
